第一項規定之文件影本，申請人應敘明與正本相符並簽章，驗證機關(構)必要時得要求提出正本供查核。
An applicant shall provide a signed declaration stating that the copies of the documents mentioned above are identical to the originals.
If necessary, the BSMI may require the applicant to present the original documents for verification.
第5條驗證登錄之申請案經審查符合者，准予登錄，
Article 5 The BSMI will grant registration and issue a registration certificate to the applicant after the application has been reviewed and approved.
發給商品驗證登錄證書，並准予依據商品檢驗標識使用辦法之規定使用驗證登錄之商品檢驗標識。
Upon issuance of the certificate, the applicant will be permitted to use the certification mark in accordance with the Regulations Governing the Use of Commodity Inspection Marks.
前項之審查，驗證機關(構)必要時得要求申請人提供樣品，就特定項目執行測試、監督試驗或派員赴生產廠場實地查核。
The inspection authority may request samples to be provided by the applicant for performing necessary testing or witness testing on specific features of the commodity.
第6條商品驗證登錄證書有效期間屆滿之日前三個月至屆滿日前，證書名義人得檢附延展申請書及相關資料申請延展，以一次為限。
Article 6 A certificate holder may apply for extension of the term of validity by providing relevant documents within a period of one to three months prior to the expiration of the certificate.
An extension is limited to one time only.
逾限申請延展者，應重新申請驗證登錄。
For applications that are made after the expiration of the certificate, a new application for registration of product certification shall be required.
前項延展之申請經審查核可者，換發證書。
A new certificate will be issued after the application for extension of the term of validity mentioned in the preceding paragraph is reviewed and approved.
前項換發證書之有效期間，自原證書有效期間屆滿之次日起算，並依本法第三十九條第二項公告之期間核計。
The extended term of validity of a registration certificate shall be determined according to that announced in Paragraph Two of Article 39 of the Act, beginning from the day following the expiry date of the original certificate.
第一項延展之申請及審查，準用第四條及第五條之規定。
Articles 4 to 5 shall apply to the application and review mentioned in the first paragraph of this Article.
第7條取得驗證登錄之商品，得逕行運出廠場或輸出入；
Article 7 Commodities that have been granted registration may be shipped out of the production premise, or be exported or imported.
其輸出入人如非商品驗證登錄證書之名義人者，得經該證書名義人之授權，取得驗證機關(構)同意放行文件辦理通關。
If the exporter or importer is not the certificate holder, they may obtain authorization from the certificate holder and import release notice from the BSMI in order to pass through customs.
第8條取得驗證登錄之商品，驗證機關(構)得派員至生產廠場、港口倉儲場、進口商、經銷商或相關處所執行取樣檢驗。
Article 8 For commodities that have been granted Registration of Product Certification, the inspection authority may dispatch inspectors to the production premises, harbor warehouse, premise of the importer or distributor to conduct sampling inspection, or to the production premises for surveillance visits.
取得驗證登錄之商品，驗證機關(構)得派員至生產廠場執行製造階段之檢查。
Article 8 For commodities that have been granted Registration of Product Certification, the inspection authority may dispatch inspectors to the production premises, harbor warehouse, premise of the importer or distributor to conduct sampling inspection, or to the production premises for surveillance visits.
第9條取得驗證登錄之商品如有變更，應依下列規定辦理：
Article 9 If any changes are made to commodities that have been granted registration of product certification, the following procedures shall apply:
一、基本設計已變更者，應重行申請登錄。
1.
Where the basic design has been changed, a new application for registration shall be submitted;
二、基本設計未變更而有其他應受檢驗項目變更之系列商品，應申請系列型式登錄。
2.
Where the basic design remains the same but the inspection items of a serial commodity have been changed, an application shall be submitted for registration of this serial commodity; or
三、前款之變更不影響證書登錄事項及商品識別者，向驗證機關(構)申請核准。
3.
Where the basic design remains the same and the changes mentioned in the previous paragraph do not affect the registered items of certificate and commodity identification, an application for such changes shall be submitted to the inspection authority for permission.
第10條取得驗證登錄之商品，其生產廠場有變更或遷移時，應向驗證機關(構)申請核准。
Article 10 For commodities that have been granted registration of product certification, an application for the change or relocation of the production premises shall be made to the inspection authority for approval.
生產廠場登載於商品驗證登錄證書者，並應同時申請換發新證書。
Where the production premises are listed on the registration certificate, an application for replacing the certificate shall be required.
第11條有下列情事之一者，註銷其商品驗證登錄證書：
Article 11 The registration certificates shall be cancelled under any of the following circumstances:
一、自行申請註銷。
1.
Where the nominal holder of the certificate applies for cancellation of the certificate;
二、驗證登錄經撤銷或廢止。
2.
Where registration of the commodity is revoked or rescinded by the BSMI.
第12條商品驗證登錄證書如有遺失或毀損者，得申請補發或換發；
Article 12 An application for a replacement of the original registration certificate may be made if the original certificate is lost or damaged.
申請系列產品登錄，其證書原登載事項有變更者，應繳回原證書，憑以增列登錄後換發新證書。
When an application for registration of serial commodities is made, the original certificate shall be returned before issuance of a replacement certificate with the addition of the registered serial commodities.
